Kiku is a trans woman. The "woman at heart" is enough explanation, at every other point the story and characters treat her as a woman. The mask could also be a subtle metaphor, additional to being really fucking cool. I think the whole community calls her by female pronouns. Also, her voice actresses in multiple languages are all women. Unironically Top 5, I love her so much.
