Thrown to the Wolves Tier 1 Variant

It was pretty fun writing Thrown to the Wolves yesterday. I actually managed to run it with my Saturday group last night, too.
I'm probably going to do up to three variants with it, each tied to one of the four tiers; the original module is tier 2.
For the first tier, what type of threat would you like to see? I've suggested a few below that I'm itching to do, but I've also included an option to suggest your own.

That'd be fun! Not sure if the bloody cave motif will fit, though. But a clock tower with twisting stairs might be interesting
