Gonna say maybe just a normal sized guy with a giant dick like way bigger than it should be for his size. His body is also showing signs of growth in other places, like an uneven growth scenario. Maybe a syringe of some fluid is nearby to show why he's growing. Dunno if that's too complicated but I figured I'd shoot my shot lol
