They have to do c-sections for very young girls because they are not old enough for the pelvic to open for the child. It’s really sad because I think the youngest to ever girl birth was 5 or so

The official translation of novel differentiated them by calling her Empress Regnant. Technically the position of Empress Regnant doesn't exist in the Li Empire (this fictional not China) as it's a patrilineal monarchy but the people addressed her as if she was, calling her Empress/"Jotei" (女帝) instead of Koutaigou/Empress Dowager (皇太后) or Mother of the Nation/Kokubou (国母).

This episode's translation by crunchyroll was really poor, as it hid important details. Without giving spoilers, there are actually 2 empresses involved: - Empress Dowager Anshi, mother of the current Emperor - simply "empress": this is the mother of the previous emperor. (Netflex's translation gave a proper title to this empress though) Let's wait and see if they would clarify it a bit in the next episode, otherwise it would be really confusing.
