Tuco was definitely Hank's first kill, otherwise it wouldn't have changed him so much. Hollywood movies and shows have distorted our view of reality, fact is that the vast majority of cops (or DEA agents in this case) never kill anyone in their whole career and never actively use their guns even once. So even statistically it would be extremely unlikely that Hank would ever have shot anyone before.
