Depending on exactly how hands are restrained, a common one I've seen is to hold onto keys or some other noisy object and use dropping them as the safe "word".

Some options can include grunting three times or shaking your head back & forth three times. Works even if you’re gagged. Tapping your (restrained) hand or foot three times is another one.
