Most NPCs (Non-Player Characters) have huge double standards when it comes to NPC versus Player character lack of wearing equipment. No NPCs in games I know of react to other NPCs not wearing anything, but if you do it, there can be all sorts of critical dialogue, and the irony in panel two is no exaggeration.
On rare occasions, however, NPCs will be in favor of it. In Fallout 4, you can actually gain some affinity with a follower named Cait by running around without anything on.

There have been a few times when am in the process of switching equipment when I wind up unexpectedly (to me) naked.

I really wouldn't. This is fully keeping in with what's in the un-modded games, and I don't really want to draw the alternative anyway.

US anti-porn regs are giving people grief. Patreon's just protecting themselves from people who don't need to go down rabbit holes to get somebody under those regs.

It's my understanding that this is common in a lot of CRPGs. There's only a couple I've encountered that have naked == naked, most of the ones I've seen that don't have naked= underwear have no naked option. They might have an underwear option, or they might have an option with no armor that has underwear, but is never mentioned.
