I was able to add my PayPal recently without issue. You need to disable R18/R18G on your Pixiv visibility settings, connect PayPal, then re-enable R18/R18G and you'll be able to use PayPal to pay for Fanbox subscriptions, Pixiv requests (no R18 requests unless your Pixiv region is set to Japan, but this is just a profile setting and doesn't actually verify you're in Japan).
