The main hidden subject of this film is Mind Control. Government type stuff like MK-Ultra. Also, Alex being co-opted by the government at the end is standard practice - Intel make useful assets out of all sorts of dodgy folks! Also the word govern-ment is Latin for 'control-mind', or mind control.
Why couldn't it be made today? There are far more violent movies around compared to this, torture etc.
